This Opportunity:
The Intensive In-Home Associate III is responsible for providing intensive family preservation in order to improve individual and family strengths, stabilize the home environment, promote positive family interaction, and prevent the use of out-of-home placement for up to twelve individuals/families.
What You'll Do:
oCoordinate and oversee the initial and ongoing assessment activities, assist the Team Leader in identifying strengths and utilizing those strengths in order to help individuals and families achieve their stated goals/dreams.
oConvene the Child and Family Team for person-centered planning.
oComplete the initial development and ongoing revision of the Person-Centered Plan and ensuring its implementation.
oConsult with identified medical (e.
g primary care and psychiatric) and non-medical (e.
g.
DSS, school, DJJDP) providers, engage community and natural supports, and include their input in the person-centered planning process.
oEnsure linkage to the most clinically appropriate and effective services including arranging for psychological and psychiatric evaluations.
Assist individuals and families in accessing community resources, including locating recreational activities, housing, food, clothing, school programs, vocational opportunities or services, providers to teach life skills, and relevant mental health, developmental disabilities and substance abuse servicesoProvide and coordinate behavioral health services and other interventions for the youth or other family members with other licensed professionals and Child and Family Team members.
oMonitor and document the status of the recipient's progress and the effectiveness of the strategies and interventions outlined in the Person-Centered Plan.
oParticipate in the person-centered planning process.
oAssist with implementing a home-based behavioral support plan with the youth and his or her caregivers as indicated in the Person-Centered Plan.
oProvide psychoeducation as indicated in the Person-Centered Plan.
oAssist the team leader in monitoring and evaluating the effectiveness of interventions, as evidenced by symptom reduction and progress toward goals identified in the Person-Centered Plan.
oAssist with crisis interventions.
Provide 24/7/365 on-call access to families served by the intensive in-home team and be a first responder to families in cases of emergencies.
oAssist the team leader in consulting with identified providers, engaging community and natural supports, and including their input in the person-centered planning process.
Assist individuals and families in developing natural resources and make contact with social support networks.
oMaintain necessary medical records in compliance with state, federal, and agency guidelines.
oAttend educational trainings/seminars to remain up to date on current best practices and maintain required certifications and competencies.
oPreserve positive working relationships within the communities served, including individuals, families, staff, monitoring and licensing agencies, organizations, funding resources, and other human service agencies.
oDrive and travel extensively to community locations, various agencies, and other outreach destinations.
Provide and/or arrange for transportation for individuals and families as required.
oParticipate in the ongoing quality improvement activities of the agency.
oFollow service definition guidelines for services being provided.
oDemonstrate knowledge of and comply with all agency policies and procedures.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
