Conducts nursing assessments to assist in the overall management of patient/caregiver needs.
When appropriate, develops, implements, evaluates, and updates the plan of care to ensure that care is uninterrupted and that goals are achieved.
Facilitates patient and caregiver independence to the extent possible.
When providing hospice care, appropriately utilizes VIA Health Partners patient related protocols, standing orders and guidelines.
Assumes responsibility for conducting effective patient and caregiver education.
Encourages and allows time for questions and consultation.
Provides education to patients and families as needed.
Communicates effectively with members of the IDG, patients, caregivers, and other health professionals to ensure continuity of care.
Maintains open lines of communication with supervisorKeeps supervisor apprised of any complex patient/caregiver issues.
Applies principles of infection controlFollows guidelines for universal precautions in accordance with accepted professional practices.
Interdisciplinary GroupAttends, participates, and may facilitate interdisciplinary group conferences IDG and special care conferences.
Assumes responsibility for establishing and maintaining effective working relationships with team members.
Identifies specific patient/caregiver needs and make referral to appropriate team members.
DocumentationEnsures accurate, complete, succinct, and timely clinical documentation in accordance with VIA Health Partners guidelines and the requirements of licensure and certification standards.
Professional and Personal AccountabilityDemonstrates competency in knowledge and skills.
Ensures that VIA Health Partners professional reputation is maintained and projected.
Assumes responsibility for professional development and staying abreast of current trends in the healthcare fieldIncorporates new information and methods into practice.
Maintains current professional licensure, CPR certification and CEU requirements.
Maintain productive and professional relations with external agencies and healthcare providers.
Exhibits time management skillsAble to plan workday while remaining able to respond appropriately to crisis.
May perform other duties as required(IPU) Cleans patient rooms as required per patient room cleaning procedureEnsures all areas of facility are clean and appropriately stocked with suppliesKeeps the family room clean throughout the dayEnsures proper chemicals are used for cleaning different surfacesEnsures protective equipment is used when working with chemicals, ie, gloves, glasses.
Minimum QualificationsA Diploma or associate degree in nursing is required; Bachelor's Degree preferred.
A current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN) is required.
The Nurse Licensure Compact (NLC) allows a registered nurse (RN) and licensed practical/vocational nurse (LPN/LVN) to have one multistate license in a primary state of residency (the home state) and to practice in other compact states (remote states), while subject to each state's practice laws and discipline.
Current CPR required; CHPN preferred.
Minimum 1-year related experience for inpatient units (IPU) and 2 years for other nursing positionsExperience in direct care of patients with life limiting illnesses requiredRelated experience may include hospice and palliative care; family medicine; internal medicine; geriatrics, oncology; home health care; medical surgical.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
